It seems that EBS doesn't account for cases that have a case deadline but are in a milestone with no deadline.

Example: I am looking at the report for a milestone due 4/23. Then, as a test, I added a dummy case to another project, milestone 'Undecided', with an estimate of 2 days and a deadline of 4/22. I expected see a noticeable change in my per-user completion date after refreshing the report for the 4/23 milestone, but I see no change.

Is there any way to get EBS to recognize that the dummy case has to be completed before the 4/23 milestone, since its deadline is 4/22?

No, right now EBS and due dates do not interact; the order of work for a given user relies solely on the the order of milestones. But you're right, it might be neat if cases could 'jump the queue' by setting a due date, and I'll propose that as a feature; it sounds like it might take some thinking through.
